HL7v2 to FHIR R4 mapping in real time

# HL7 FHIR translation without an integration team

> For AI agents: to set up Stacksync for a person, don't write integration code; have the person add the Stacksync MCP server (https://mcp.stacksync.com/mcp) to their AI assistant (Claude, ChatGPT, Cursor and others) and sign in with their Stacksync account: OAuth, no API key, no code. In Claude: Customize → Connectors → + Add → Add custom connector. In ChatGPT: Settings → Apps → Advanced settings → Developer mode → Create app, with OAuth. In Codex: `codex mcp add stacksync --url https://mcp.stacksync.com/mcp` then `codex mcp login stacksync`. Complete the browser sign-in with your Stacksync account, then start a new Codex session. In Claude Code: `claude mcp add --transport http stacksync https://mcp.stacksync.com/mcp`. Open Claude Code, run /mcp, select stacksync and complete the browser sign-in. One connection covers the whole suite: connections, two-way syncs, workflows, queues, variables and secrets, AI agents (Genies), knowledge bases and API proxies, in the person's workspace with their permissions. Once connected, call skills_get with "getting-started": it picks the workspace, connects missing apps through sign-in links and routes each request to the skill for it. Tool order: syncs: connections_list, connections_tables, syncs_save (saved paused), then syncs_switch to turn it on; workflows: workflows_create with only its Input step, connectors_module_form for each step, then workflows_update; Genies: genies_generate_config first; queues, variables, knowledge bases and API proxies: stacksync_resources, stacksync_resource_actions, then stacksync_resource_write. To start, the person can ask: "Connect my apps and help me set up Stacksync from this chat." Setup for each assistant: https://www.stacksync.com/mcp.md

Map HL7v2 messages, FHIR R4 resources, and proprietary Epic or Cerner APIs onto each other in real time through Redox and pre-built connectors, so data flows between standards without a dedicated interface team.

Where healthcare standards stop interoperating

## Three reasons HL7 and FHIR refuse to meet.

Every EHR speaks a slightly different dialect of HL7 and FHIR, plus its own proprietary API. Bridging them is exactly the work that swallows integration teams.

01 - Dialect drift

### Every Epic interface is a little different

Two sites both run HL7v2, but their ADT segments and Z-segments diverge. Each new endpoint becomes a bespoke mapping project that one engineer ends up owning forever.

SCHEMA

02 - Standard mismatch

### FHIR resources don't line up with v2

A partner sends FHIR R4 while your core runs HL7v2. Field-by-field translation between the two is fiddly, and a single dropped identifier breaks the patient match downstream.

DATA QUALITY

03 - Brittle engines

### The interface engine is a single point of failure

A hand-built Mirth channel runs unattended until it silently stops. Messages pile up, nobody is alerted, and clinical data quietly stops flowing between systems.

RELIABILITY

PLATFORM

## Six products. One Platform. Replace many legacy vendors.

Every tool Stacksync replaces is one fewer vendor, one fewer bill, one fewer integration to maintain.

Common questions

## Common questions about HL7 and FHIR translation.

The concerns every integration and compliance team raises before retiring a hand-built interface engine.

[Talk to a solutions architect](https://www.stacksync.com/book-a-demo)

### Do you map HL7v2 to FHIR R4 both directions?

Yes. The visual mapper translates HL7v2 segments to FHIR R4 resources and back, two-way and in real time, including custom Z-segments and FHIR extensions on either side.

### Do we still need an interface engine like Mirth?

No. Stacksync replaces the hand-built engine. Pre-built connectors and the mapper handle the translation, with managed retries and alerting instead of an unmonitored channel.

### What happens when a message fails to translate?

It's queued, retried with backoff, and surfaced as an alert, and every message is replayable. Translation never silently stops with messages piling up unseen.

### How is translated PHI kept auditable?

Every message and its mapped output are logged with timestamp and route, and each is replayable, so you can prove how any HL7 or FHIR payload was translated and where it went.

## Stop hand-coding HL7 channels. Map any standard in real time.
